Millicent’s tale is a compelling yet tragic NPC questline in Elden Ring that players must complete to unlock one of the game’s multiple endings. The Tarnished will first encounter her within the Church of the Plague up atop the hill east of Sellia, Caelid’s Town of Sorcery. She will not have much to say, only revealing she suffers from Scarlet Rot. From here, the player cannot do anything to or for her to progress her questline. Instead, they must speak to another NPC to trigger the beginning of Millicent’s tale. This NPC is known as Gowry, the father of the same rot-stricken young woman.
How to Complete Millicent’s Quest in Elden Ring
To start Millicent’s quest in Elden Ring, the player must speak with her father Gowry in Gowry’s Shack, south of Sellia. He will request the player retrieve a needle from the swamp nearby. A Field Boss named Commander O’Neil will drop it. Afterward, return the needle to Gowry, who will fix it and ask it be delivered to Millicent.

After giving the needle to Millicent, reload the area and speak with her again. Next, return to Gowry’s Shack to find Millicent trying to regain her memories. Reload the location again to encounter Gowry, who will become a Spell merchant once his dialogue has been exhausted. Millicent will relocate to the Atlus Plateau, near the Erdtree-Gazing Hill. After speaking with her, head to the Shaded Castle in the region’s north and retrieve the Valkyrie’s Prosthesis from the eastern tower. Return the item to Millicent to progress her quest in Elden Ring.
The next step is defeating the Godskin Apostle of Windmill Village, northwest of the Shaded Castle. Talk to Millicent to progress her storyline. Once players have made it to the Mountaintops of the Giants, they will run into Millicent again at the Ancient Snow Valley Ruins. Next, Elden Ring fans must gather the two halves of the Haligtree Secret Medallion to reach Elphael, Brace of the Haeligtree, where they will meet Millicent within the Prayer Room.
To advance her story, venture into the under-tree zone of the Legacy Dungeon and beat the Ulcerated Tree Spirit. Once this is done, Tarnished can choose to either invade Millicent or be summoned to help her fight her sisters. To receive the quest item needed for the ending, aid her in battle to defeat her sisters. She can then be found beside the pool where Tree Spirit spawned. Finally, Millicent will present the player with the Unalloyed Gold Needle, which must be used on the giant flower after defeating Malenia.
