Getting your hands on the Rotten Winged Sword Insignia is a long process that will take a majority of the game to actually complete. This is because it is a reward for one of the endings to the questline for the NPC Millicent, who you will find in several locations across the Lands Between. First, players will need to speak to father Gowry in Gowry’s Shack, found in Caelid to the south of Sellia. Then, defeat the boss named  Commander O’Neil in the nearby swamp to get the Unalloyed Gold Needle. Return it to Gowry, reload the area and bring the needle to Millicent at the Church of the Plague. This is the start of the journey that will reward you with the Rotten Winged Sword Insignia.

You will next find her at the Altus Plateau, near the Erdtree-Gazing Hill Site of Grace. Head to the Shaded Castle and find the Valkyrie’s Prosthesis in the castle’s eastern tower. You will now need to kill the Godskin Apostle boss at the Windmill Village. Find both halves of the Haligtree Medallion and gain access to the Consecrated Snowfield and then the Haligtree.
